Scars after caesarean section: What microstimulation can do

Reading time: 5 Min.

For many women, a caesarean scar is more than just a cosmetic question. Numbness above the scar, a pulling sensation in the lower abdomen when sitting up, a visible ridge or an “overhang” above it: all of this can still be present years after birth and affect everyday life.

Why caesarean scars need special attention

In a caesarean section, several tissue layers are cut: skin, subcutaneous fat tissue, fascia, muscle, peritoneum, and the uterine wall. During healing, tissue at the cut line can adhere, individual layers remain stuck together at points where they should actually glide.

Possible consequences:

  • Pulling sensations in the lower abdomen, especially when standing up or moving
  • Numbness above the scar, sometimes also above or below
  • A visible “overhang” (scar keloid or adherent ridge)
  • Pelvic floor dysfunction due to altered fascial tension
  • Psychosomatic aspects: the scar as a permanent reminder of an often unexpected birth situation

How microstimulation works on scar tissue

Mechanical vacuum microstimulation (as used by icoone® Roboderm®) acts on scar tissue at several levels:

  • Mobilising adherent layers: superficial scar adhesions between skin and subcutaneous tissue can be loosened
  • Activating microcirculation: scar tissue is often poorly supplied with blood; better circulation supports tissue repair
  • Stimulating fibroblasts: promotes collagen and elastin remodelling in the scar area
  • Lymphatic drainage: reduces swelling and congestion in surrounding tissue

Important delimitation: Deeper adhesions in the abdominal cavity (intraperitoneal adhesions, e.g. between uterus and abdominal wall) cannot be directly dissolved by external microstimulation. For such findings, medical specialist disciplines are responsible.

When treatment makes sense

Earliest: As soon as wound healing is fully complete and medical clearance is given. Usually about 6 to 8 weeks after the caesarean. Individual healing can vary, consultation with the gynaecologist or midwife takes precedence.

Optimal: In the first 12 to 18 months after surgery, scar tissue is still in active remodelling. Treatment in this phase is most effective.

Also possible later: Even years or decades after the caesarean, microstimulation can improve texture and sensation in the scar area, though less strongly than with fresh scars.

Scientific basis

Mechanical stimulation activates fibroblasts via mechanotransduction and can stimulate the production of collagen, elastin, and hyaluronic acid (Humbert et al., Clinical Interventions in Aging, 2015). For the icoone® technology itself, specific clinical studies are available, including on post-operative rehabilitation and scar treatment. These are accessible as PDFs at icoone.com/en/science.

icoone® Medical is CE-certified as a medical device and is explicitly recommended by the manufacturer for scar treatment. This is not equivalent to a pharmaceutical approval but means: the device meets the requirements of the EU Medical Device Regulation for its intended use.

Realistic expectations

What is achievable:

  • Visibly softer, more mobile scar
  • Reduction of pulling and tension sensations
  • Improved circulation and skin sensation (also with numbness)
  • Less pronounced ridge, if present at all
  • Better integration of the scar into surrounding tissue

What is not achievable:

  • Complete disappearance of the scar
  • Dissolution of deep adhesions in the abdominal cavity
  • Immediate results after a single session

Typical treatment course

  • First session: detailed history, gentle first treatment to acclimatise
  • Course phase: 10 sessions 3 to 7 days apart
  • Maintenance: after the course, once monthly to stabilise results

Sensible in parallel: – Postnatal exercises for pelvic floor or abdominal complaints – Gentle self-massage of the scar with a high-quality oil (e.g. jojoba or almond oil) – Adequate hydration to support lymph flow

When to see a doctor

A caesarean scar belongs in medical evaluation when:

  • Redness, warmth, purulent discharge appear (signs of infection)
  • Strong, persistent pain exists
  • A hernia (scar hernia) is visible (bulge under the scar when pressing)
  • Unusual skin changes appear

In such cases, the scar belongs first in gynaecological or surgical hands before aesthetic or functional treatments are considered.

Frequently asked questions

When after the caesarean can I start scar treatment?

Usually from 6 to 8 weeks after the operation, as soon as wound healing is fully complete and medical clearance is given. Every healing is individual, consultation with the gynaecologist or midwife takes precedence.

Can old caesarean scars still be treated?

Yes. Scars years or decades old also benefit from microstimulation, though less strongly than fresh scars. In the first 12 to 18 months after surgery, tissue is most malleable.

How does treatment over the scar feel?

Pleasant to neutral. Many women describe a slightly pulling, releasing sensation, especially with adherent scars. Treatment should not be painful, intensity is adjusted if there’s discomfort.

Does icoone® also release internal adhesions?

Superficial scar adhesions can be loosened. Deep intraperitoneal adhesions (e.g. between uterus and abdominal wall) cannot be directly dissolved by external microstimulation. For such findings, medical specialist disciplines are responsible.
